Medication Forms:

• Only students who are bringing medication on the trip need to turn in a medication form.
• IF a student already has medication in stock and on file at the school, NO FORM IS NEEDED.
• All medication forms must be signed by a physician.

General Medication Form

• Fill out one per medication.
• Each form needs a doctor's signature.
• This form is for any medication your child may take during the trip.

"Tylenol" Form

• This form ONLY allows for school-supplied Acetaminophen tablets in 650 mg doses to be given to students.
• This form CANNOT be accepted if the dosage or prescription information is altered in any way.
• It MUST be signed by a physician
